On October 13, 2021, an employee was operating a wire draw machine. During the drawing operation, the employee was adding lubrication powder to the die area of the machine when the glove on their right hand was caught and pulled into the rotating spindle. The pinch point where the wire wrapped around the spindle caused a partial amputation of the employee's third digit and a severe laceration to the fourth.

On August 30, 2021, an employee was unpacking and redistributing assembly materials. He used a scalpel to cut the banding on a bundle of vertically placed steel L-profiles. The L-profiles then fell over and onto the employee. The employee was hospitalized with fractures to both legs.

An employee was standing on the platform at the galvanizer machine holding onto the metal hand rail and observing the moving shovel/ladle for cleaning the galvanizer. The shovel/ladle struck the middle divider of the holding tank causing the shovel/ladle to move up and down roughly. As a result the employee's hand was struck-by the handle of the shovel/ladle and their fingertip was caught between the metal hand rail and the metal handle and amputated.

An employee was operating an enclosed pallet jack and stopped to scan a pallet. As the employee reached out to scan the pallet, the cord of the scanner hit the throttle of the pallet jack. The pallet jack lunged forward and pinched the employee's right leg against a doorway protection pole resulting in a lower leg fracture.

An employee was loading liquid fertilizer from the safety rack onto a tank that was on the trailer of a truck. The truck moved and pinched the employee's leg between the safety rack and the trailer. The employee was hospitalized with two broken bones in the right leg, located 14 inches above the ankle.
